When you’re thinking about breast augmentation, it’s easy to obsess over choosing the perfect size or type of implant — but one of the most important decisions is actually where the implant goes. During your consultation, we’ll talk about whether placing your implants above or below the chest muscle makes the most sense for your body, your lifestyle, and the look you want.

Let’s break it down in simple terms.

Above the Muscle (Subglandular) Breast Implant Placement

This option places the breast implant over your chest muscle but under your natural breast tissue (subglandular). This can create a fuller, more rounded look, especially if you already have a moderate amount of breast tissue to begin with.

The pros? Recovery is often a little easier than below-the-muscle placement, and your implants won’t move when you flex your chest muscles (a bonus if you’re into weightlifting or yoga). The cons? If you’re very lean or have little natural tissue, the implant might be more visible, and you could have a slightly higher risk of rippling and capsular contracture (a complication where scar tissue hardens around the implant).

Below the Muscle (Submuscular) Breast Implant Placement

With this option, the implant goes partially underneath the pectoral muscle. It’s a great choice if you want a softer, more natural slope — especially if you’re starting with less breast tissue.

The pros? Submuscular placement lowers the risk of capsular contracture, and the implant is less visible — a huge benefit if you had a smaller bust beforehand. The cons? Recovery can be a bit more intense at first, and your implants may move slightly when you flex. But all in all, most patients say it’s well worth it to get below-the-muscle placement.
